Option 1: Schedule an In-Office Appointment Online

Prior to Starting, You Will Need:
- Insurance cards (if applicable)
- Access to your email (for security purposes, you will receive an email with a 4-digit security code to enter during scheduling).
To schedule your appointment at your convenience, please click the floating blue “Request Appointment” button in the bottom right corner of the webpage. If you do not see an appointment that works with your schedule or need to schedule a procedure, please call the office to confirm availability.
Online Appointments can only be scheduled with Dr Azhar and Aesthetician Jaelin at this time.

Appointment Types & Single Concern Visits: Skin cancer screenings, acne, rashes, and hair loss each require dedicated, focused evaluations and must be scheduled individually.

Hair Loss Appointments: Hair loss consultations require a specific appointment type to allow sufficient evaluation time. If hair loss is not specifically scheduled under the correct appointment type, your visit will be rescheduled on-site upon arrival.
Multiple Concerns: If you have multiple concerns (e.g., acne and a skin cancer screening), please select your primary concern for this visit. Additional concerns will be addressed at a follow-up appointment. Please do not book back-to-back appointments, as duplicate slots are automatically canceled.
Appointment Types: Skin cancer screenings, acne, rashes and hair loss require detailed, focused evaluations. A dedicated visit is required to thoroughly address these complaints. For example, if you have concerns about both acne and would like a skin cancer screening, you will be asked to defer the screening to take place at your follow up appointment. Please do not book two separate appointments back-to-back, as one will be automatically cancelled.

*APPOINTMENT CONFIRMATION REQUIRED: You will receive an initial email regarding your appointment request followed by a second confirmation email once your appointment has been confirmed by our office. If you have not received a second confirmation email within 3 business days, your appointment has not been scheduled. Please call our office to confirm your appointment.

⚠️ Important Notice Regarding Hair Loss Consultations: Hair loss evaluations require specialized scheduling and dedicated room time. For first-time hair-loss evaluations, please call our office to schedule your appointment. If a hair loss visit is not scheduled properly in advance, the appointment will be rescheduled on-site.
