INSURANCE

What insurances do we accept? 

We accept most major insurance. Prior to your visit, we recommend that you verify with your insurance company whether we are in-network with your particular plan.  The following is a list of insurances we accept; we will make updates as necessary.
*Please note some individual plans may be excluded even if they fall under the following insurance companies.  It is the patient’s responsibility to verify coverage with your carrier prior to your appointment.

APPOINTMENTS

Do we need a referral from your doctor? 

If your insurance plan is an HMO, you will be required to obtain a referral from your primary care physician in order to be seen in our office.  It is the patient’s responsibility to ensure that this has been completed and the referral received by our office prior to your office visit.  Please be advised, your visit may have to be rescheduled if we do not have a copy of your referral authorization.

Patients who have PPO insurance; are self-pay; or are enjoying our cosmetic offerings do not require a doctor referral.

Where can I find the online paperwork to fill out before my appointment? 

Please check your email for an email verification link to set up your username and password for your Sadio patient portal. If you are unable to locate this email, you can do a search in your mailbox for “Sadio”. After you have set up an account, go to https://sadio.com/login and enter your username and password to access your registration paperwork. 

Do we accept walk-ins? 

We require patients to make an appointment before being seen by the doctor. Same-day appointments are frequently available. Please call our office at 469.480.8100 to check availability.  

How long should I expect to be at the office for my visit?

We strive to get patients in and out of the office in about an hour. There may be a delay if there is an unexpected situation with a patient, but we value your time and prioritize seeing you promptly. 

Do you offer virtual appointments? 

We do not offer virtual appointments at this time. Our aesthetician is often able to accommodate a virtual consultation.

What conditions do you treat?

At Affinity Dermatology, we are a medical, surgical, and cosmetic dermatology practice with a variety of modalities to address the dermatologic needs of patients at any part of the age spectrum, and with any hue of skin. 


DIRECTIONS TO OUR OFFICE

PLEASE DO NOT RELY SOLELY ON GPS. Please read the DRIVING DIRECTIONS below. GPS maps are navigating to the THR hospital because this is a new development. If you end up here, please re-enter the feeder road driving north until you reach us.

DRIVING DIRECTIONS:

Driving NORTH on the Dallas North Tollway from PLANO/FRISCO: Exit Panther Creek. You will pass the new Texas Health Resources/UTSW hospital and the grey/white Frisco Medical Pavilion complex on your right. The next complex on your right with one-story brown and tan medical offices (it is still under construction, but we are in a completed suite), is where we are located. Turn right into the complex, and drive to the back of the complex. Our clinic is in the northernmost building that faces the houses. We are located in Suite #520.

Driving SOUTH on the Dallas North Tollway from PROSPER/CELINA: Exit Eldorado, Remain left and go through the U-turn at Eldorado to go North on the feeder road. You will pass the new Texas Health Resources/UTSW clinic and the grey/white Frisco Medical Pavilion complex on your right. The next complex on your right with one-story medical offices (it is still under construction, but we are in a completed suite), is where we are located. Turn right into the complex, and drive to the back of the complex. Our clinic is in the northernmost building that faces the houses. We are located in Suite #520.
