Preparing For Your Visit

We look forward to your upcoming appointment at Vital Dermatology. In order to coordinate your care safely & effectively, please complete and review the following:

*REQUIRED* Complete Patient Paperwork on our Portal:

*The standard in-office check-in is completed on an iPad. In order to maintain a limited lobby capacity, you are required to complete the check-in process on the portal anytime prior to your appointment.

  • Verify your email: You should have received an email from noreply@ezderm.com to sign up for our patient portal.

  • Create a username & password

  • Answer all intake questions

  • SIGN THE CONSENT FORMS (*Under "Appointments" at the TOP RIGHT)

COVID-19 policy

Please note our community is experiencing a surge of COVID-19 cases & our office is adhering to strict COVID-19 precautions. We are unable to make exceptions to our protocols.

  • Masks required, even if you have been vaccinated.

  • Guests, including children, are not permitted unless the patient is a minor or has a physical or mental impairment.

  • Please reschedule if you have cold/flu symptoms (even if you have had a negative test), exposure to COVID-19 or a positive COVID-19 test within 2 weeks of your appointment. Symptoms include: fever, cough, sore throat, runny nose, loss of taste/smell, diarrhea or body aches. You must be symptom free of cold/flu or COVID-19 to be seen regardless of testing result. If someone in your home is COVID-19 positive, you may not be seen for 28 days.

  • Vaccination is highly recommended prior to your appointment.

For your examination:

  • Remove all make-up prior to your appointment

  • Do NOT wear lotion or moisturizer to your appointment

Insurance and Payment:

  • Bring your ID and insurance card(s). We encourage you to call your insurance company to ensure we are in your network.

  • Payment is due at time of your appointment for services performed and may co-pay, co-insurance &/or unmet deductible depending on your plan's benefits.

  • Credit card on file is required for all patients but is only utilized in the event of a patient refund or delinquent payment.

No Show and Late Appointment Policies

*No-Show Policy: In the event that you need to cancel or reschedule your appointment, please call 24 hours prior or a no-show fee will apply.

*Late Appointment Policy: Patients who arrive more than 10 minutes after their scheduled appointment time will kindly be asked to reschedule.
