What to bring with you for your first visit:
- List of medications with dosages. Do not forget to include over-the-counter medications, herbal supplements, vitamins, creams, forms of hormonal treatments, including birth control, and previously prescribed medications for your condition.
- List of drug allergies
- Photos of conditions that change over time are welcome. We suggest that you put them in a special folder for easy locating during your visit.
What to Wear for your first visit:

- Please wear loose, comfortable clothing to allow for the best evaluation of the area of concern.
- Remove makeup prior to your visit for evaluation of face skin.
- Remove hairpieces if scalp evaluation is needed (including sew-in extensions, crochet, braids that obstruct the view of the scalp, and camouflage hair fiber products).
- Please come as your skin normally looks with regular care (clean, dry hair; follow your normal skincare regimen).
How to prepare for your first visit:
- Please complete intake paperwork on our secure Sadio patient portal before your appointment. You have been emailed a link from Sadio to sign up for your account. Please do not forget to sign consent forms under “Appointments” at the top right.
- There will be some consents that you will fill out on an iPad when you come in.
- Please bring a valid ID insurance card and driver’s license. Double-check to be sure that your insurance is contracted with our clinic.
- We ask patients with a cough or sneezing to wear a mask during their visit; one can be provided upon request. You may be asked to reschedule if you appear visibly ill or have a fever. Your health is our utmost priority.
- If you’re getting laser hair removal, please shave the night before. Discontinue retinol/retinoid medications (retinol, adapalene, tretinoin, trifarotene, tazarotene) 5-7 days prior to treatment or as per the advisement of your provider.
